Weight Loss Injections in Pakistan: Doctor-Led Programme

Lost weight before, only to see it come back? Weight loss injections are weekly prescription medicines, such as semaglutide and tirzepatide, that help you feel full on less food. We prescribe them only after a doctor’s review, as part of a plan with eating and activity coaching, and they start from Rs 6,995 a dose.

Best for adults whose weight affects their health, years of diets that didn’t last, or weight that keeps coming back.

How do weight loss injections work?

Weight loss injections are weekly prescription medicines that copy the hormones your gut releases after a meal. They tell your brain you are full, slow how fast your stomach empties and steady your blood sugar, so smaller portions feel like enough and cravings quieten. The two used for weight loss are semaglutide, the medicine in Ozempic and Wegovy, and tirzepatide, the medicine in Mounjaro.

They work best as part of a plan. In the main trials, people also ate less and moved more, with regular support, which is why every injection we prescribe comes with a dietitian or lifestyle coach.

Mounjaro vs Ozempic: which is better for weight loss?

Both are once-a-week pen injections, and both work. The main difference is how strongly.

Ozempic / WegovyMounjaro
MedicineSemaglutideTirzepatide
How it worksCopies one gut hormone (GLP-1)Copies two gut hormones (GLP-1 and GIP)
Weight lost in its main trialAbout 15% over 68 weeks15–21% over 72 weeks, by dose
Head-to-head trial13.7% over 72 weeks20.2% over 72 weeks
Starting dose0.25 mg a week, raised every 4 weeks2.5 mg a week, raised every 4 weeks

In the first trial to compare them directly (SURMOUNT-5, 751 adults with obesity and without diabetes), people on tirzepatide lost 20.2% of their weight on average over 72 weeks, against 13.7% on semaglutide. Mounjaro’s maker, Eli Lilly, ran the trial, and both groups were also coached on diet and activity.

Stronger isn’t always better for you. Your doctor weighs your health, your other medicines and how your stomach copes, and many people do very well on semaglutide. In Pakistan, semaglutide and tirzepatide are each sold under more than one brand name, so your doctor tells you exactly which product and dose you are prescribed: see semaglutide injection and tirzepatide injection.

Who can have weight loss injections?

They are for adults whose weight affects their health, not for losing a few kilos before an event. Their licences abroad cover adults with a BMI of 30 or more, or 27 or more with a weight-related health problem such as high blood pressure, type 2 diabetes or sleep apnoea.

Health risks start at a lower weight for South Asian people, so guidelines such as the UK’s NICE use lower cut-offs for them, with obesity from a BMI of 27.5. Your doctor looks at your waist, blood sugar, blood pressure and history too, not only the number.

Work out your BMI: your weight in kilograms divided by your height in metres, squared. For example, 85 kg at 1.65 m tall: 85 ÷ (1.65 × 1.65) = 31.2.

They are not prescribed in pregnancy or while breastfeeding, or if you or a close relative has had medullary thyroid cancer or MEN 2 syndrome. Past pancreatitis, gallbladder disease, severe stomach-emptying problems, or diabetes medicines that can cause low blood sugar all need a careful review first.

Side effects of weight loss injections

The most common are stomach side effects: nausea, diarrhoea, constipation, vomiting and indigestion. They are usually mild to moderate, come mostly in the first weeks and after each dose rise, and ease with time. In the head-to-head trial, fewer than 6 in 100 people stopped because of them (5.6% on semaglutide, 2.7% on tirzepatide).

Rarer but serious are inflammation of the pancreas, gallstones, dehydration from vomiting, and low blood sugar if you also take some diabetes medicines. Get help at once for severe stomach pain that doesn’t go away, especially if it spreads to your back.

How we keep side effects down:

  • Start low and raise the dose slowly, no faster than every 4 weeks
  • Eat smaller meals, slowly, and stop when you feel full
  • Drink plenty of water and go easy on greasy, heavy food
  • Regular check-ins, so your doctor can hold or lower a dose

How to spot fake Ozempic and Mounjaro pens

Demand for these injections has brought fakes. DRAP, Pakistan’s medicines regulator, has warned that falsified versions were circulating, including online, and that fakes may not work or may be toxic. The WHO has also warned of fake Ozempic pens found even in the regulated supply chain in the UK, Brazil and the US.

Signs a pen may be fake, from the WHO’s alert:

  • A dose scale that extends out from the pen when you set the dose
  • A poor-quality label that doesn’t stick well to the pen
  • Spelling mistakes on the box

Buy only with a prescription, from a licensed pharmacy or clinic, never from social media or a seller who doesn’t ask about your health. If something seems wrong, don’t use it: speak to a doctor or pharmacist and report it to DRAP.

What our weight loss programme includes

We don’t hand out pens. Our programme is doctor-led, with a dietitian or lifestyle coach beside you from the first visit.

  1. Free consultation: a dietitian or lifestyle coach goes through your weight history, routine and goals, and records your weight, waist and BMI.
  2. Doctor’s review: your health, medicines and any test results. An injection is prescribed only if it suits you and is safe for you.
  3. Your eating plan: one that fits your routine and the food you eat at home, not a crash diet.
  4. Weekly injection and dose plan: a written plan for each step up, and what to do about side effects.
  5. Regular check-ins: your weight, waist and side effects, with the plan adjusted as you go.
  6. A plan for afterwards: weight often returns when injections stop. A year after stopping semaglutide in its main trial, people had regained about two-thirds of what they lost, so we plan the step down with you and keep coaching.

Would you rather not take medicine? Lifestyle coaching on its own suits many people. Either way, your free consultation sets out the whole programme and its price before you start.

Choosing a treatment 4

01

How does medical weight loss work?

A doctor reviews your health, weight history and medicines, then builds a plan around you. Every plan includes lifestyle coaching, and a weekly injection is added only if it suits you. In a review of 27 trials, adults on structured behavioural programmes lost more weight than those without support.

02

What is the best weight loss injection: Mounjaro or Ozempic?

For weight loss alone, tirzepatide, the medicine in Mounjaro, is stronger: in a head-to-head trial, people lost 20.2% of their weight on it over 72 weeks, against 13.7% on semaglutide, the medicine in Ozempic. The best one for you depends on your health, how you tolerate it and your budget, so your doctor decides with you.

03

Can I get Ozempic, Wegovy or Mounjaro in Pakistan?

Only on a doctor’s prescription, after a full review. They are prescription-only weekly injections for adults with obesity, or who are overweight with a weight-related health problem. DRAP warns against fakes sold online, so never buy a pen from social media or a seller who doesn’t ask about your health.

04

Do slimming drips help?

Only as a small extra. Slimming drips are doctor-checked support inside a weight plan; they don’t burn fat on their own, and lasting results come from the habits you keep and, where it suits you, a weekly injection.

Cost & results 3

05

How much do weight loss injections cost in Pakistan?

Semaglutide injection starts at Rs 6,995 a dose and tirzepatide injection at Rs 12,495 a dose, both given weekly, and the first four weekly doses cost Rs 22,384 or Rs 39,984 as a course. Ozempic or Wegovy starts at Rs 45,000 for the first month. Prices are the same at all 12 clinics, and your consultation and doctor’s review are free.

06

How fast will I lose weight?

Steadily. Coaching aims for about 0.5–1 kg a week, and weight loss with an injection builds over months as the dose rises. In the main trials, people lost around 15% of their weight over about 16 months. Crash diets tend to come back; a steady plan doesn’t.

07

Will the weight come back after I stop the injections?

It can. A year after stopping semaglutide in its main trial, people had regained about two-thirds of what they lost. That is why every plan includes coaching, and why we plan the step down with you instead of stopping suddenly.

Safety & recovery 3

08

What are the side effects of weight loss injections?

The most common are nausea, diarrhoea, constipation and indigestion, mostly in the first weeks and after each dose rise, and they usually settle. Rarer but serious are pancreatitis and gallstones. Starting low, raising the dose slowly and regular check-ins keep side effects down.

09

Who shouldn’t have weight loss injections?

They aren’t prescribed in pregnancy or while breastfeeding, or if you or a close relative has had medullary thyroid cancer. Past pancreatitis, gallbladder disease or severely slow stomach emptying needs a careful review first, so your doctor checks your full history before prescribing anything.
