What is semaglutide and how does it work?

Semaglutide is a medicine that copies GLP-1, a hormone your gut releases after a meal. It tells your brain you are full, slows how fast your stomach empties and steadies blood sugar, so cravings quieten and smaller portions feel like enough. It is the same active medicine as in Ozempic and Wegovy, but ours is not made by Novo Nordisk: it is a DRAP-registered injection from a leading pharmaceutical company, sold under its own name. It doesn’t burn fat directly; it helps you eat less, which is why it works best with an eating and activity plan.

Semaglutide injection benefits

  • Fewer cravings: appetite calms, so smaller meals feel satisfying.
  • Once a week: one quick injection, on the same day each week.
  • Steady, gradual loss: weight comes off over months, with your doctor watching.
  • A lower monthly cost: the first four weeks cost Rs 28,000.
  • Steadier blood sugar: it also lowers raised blood sugar as weight comes down.
  • Doctor-led support: dose steps, side effects and coaching planned with you.

Semaglutide injection for weight loss

It is for adults whose weight affects their health, not for losing a few kilos before an event. Licences abroad cover adults with a BMI of 30 or more, or 27 or more with a weight-related problem such as high blood pressure, type 2 diabetes or sleep apnoea. Health risks start at a lower weight for South Asian people, so guidelines such as the UK’s NICE use lower cut-offs, with obesity from a BMI of 27.5. Your doctor also looks at your waist, blood sugar and history, and pairs the injection with lifestyle coaching.

Semaglutide before and after

The first weeks are about settling on the lowest dose, so most people notice a smaller appetite before the scale moves much. Loss then builds as the dose rises. In the STEP 1 trial of weekly semaglutide, at a 2.4 mg dose with lifestyle support, 86% of adults lost at least 5% of their weight over 68 weeks and about half lost 15% or more; results vary from person to person. Our patients share their own stories in the videos below.

Who shouldn’t have semaglutide?

It isn’t for you if you are pregnant, breastfeeding or hoping to conceive within two months, or if you or a close relative has had medullary thyroid cancer or MEN 2 syndrome. Past pancreatitis, gallbladder disease, severe stomach-emptying problems, diabetic eye disease, or diabetes medicines that can cause low blood sugar all need a careful review first.

Your semaglutide plan, step by step

  1. Free consultation: our team records your weight, waist and BMI and talks through your history and goals.
  2. Doctor’s review: your health, medicines and any tests; it is prescribed only if it suits you and is safe for you.
  3. Starting dose: 0.25 mg once a week, a quick injection under the skin of the belly, thigh or upper arm.
  4. Step by step: the dose rises no faster than every four weeks, and only while you are coping well.
  5. Check-ins and coaching: your weight, waist and side effects reviewed regularly, with coaching beside you.

Results and side effects

Weight comes off gradually across several months, and most people find the smaller appetite easy to live with once the first weeks pass. Nausea, diarrhoea, constipation, vomiting and headache are the most common side effects; they are usually mild, come mostly after each dose rise and ease with time. Weight tends to return when the injections stop, so your doctor plans a gradual step-down and keeps your coaching going.

Semaglutide: your questions answered

Semaglutide injection starts at Rs 7,000 a dose for 0.25 mg. The 0.5 mg dose is Rs 9,000, 1 mg is Rs 11,500 and 2 mg is Rs 14,500. As you take one dose a week, the first four weeks cost Rs 28,000. Prices are the same at all 12 clinics, and your consultation is free.

It contains the same active medicine. Ozempic and Wegovy are Novo Nordisk’s brands of semaglutide; ours is a DRAP-registered semaglutide injection from a leading pharmaceutical company, sold under its own name and not made by Novo Nordisk. So it is a different product, from a different maker, at a lower price. If you prefer the Novo Nordisk pens, see Ozempic and Wegovy injections.

It has been studied in large trials, but it isn’t risk-free, so it is prescribed only after a doctor’s check. Ours is a DRAP-registered product, not a compounded mix. Avoid online sellers who don’t ask about your health: DRAP, Pakistan’s medicines regulator, has warned that falsified versions of these medicines are sold online and may not work or may be harmful.

Nausea, diarrhoea, constipation, vomiting and headache are the most common, in women and men alike. They are usually mild, come mostly in the first weeks and after each dose rise, and ease with time. Rarer risks include pancreatitis, gallstones and dehydration. It caused thyroid tumours in rodents, so it isn’t used with a personal or family history of medullary thyroid cancer or MEN 2.

Semaglutide injection is prescribed at all 12 Cutera Aesthetics clinics in Karachi, Lahore, Islamabad, Rawalpindi, Faisalabad, Hyderabad and Sialkot, at the same price everywhere, with female doctors at every clinic. Find your nearest clinic, or message us on WhatsApp and we’ll book you in at the one closest to you.

Usually not much in the first month, because you start on the lowest dose while your body adjusts; a smaller appetite is often the first change you notice. Loss builds as the dose rises over the following months, and it varies from person to person. Your doctor tracks your weight and waist at every check-in rather than promising a number.

Everyone starts on 0.25 mg once a week for four weeks, then usually moves to 0.5 mg. If you need more help with appetite and are coping well, your doctor may raise it to 1 mg, and 2 mg is the highest dose we use. The dose never rises faster than every four weeks, and many people do well on a lower one.

Both are weekly injections; tirzepatide copies two gut hormones instead of one. In a head-to-head trial run by Mounjaro’s maker, people on tirzepatide lost 20.2% of their weight over 72 weeks, against 13.7% on semaglutide. Semaglutide starts at a lower monthly cost, and many people do very well on it. Your doctor helps you choose; see tirzepatide injection.

Semaglutide also comes as a daily tablet, taken on an empty stomach with a sip of water, and the tablets are licensed mainly for type 2 diabetes. We prescribe the weekly injection. Metformin (Glucophage) is an older diabetes tablet with a much smaller effect on weight, and your doctor tells you whether it has any place in your plan.

Before a wedding, yes, as part of a longer plan: start five to six months ahead, so the dose has settled and early side effects have passed. It isn’t used in pregnancy or while breastfeeding. Stop at least two months before trying to conceive, as it stays in the body for weeks, so if you hope to start a family soon after marriage, plan the timing with your doctor.

Yes, it works the same way in men and women: by calming appetite, so you eat less without a constant fight. Around menopause, when weight often becomes harder to shift, it can help in the same way, though it doesn’t treat hot flushes or other menopause symptoms. Either way, your doctor checks your health and medicines first.
